The 1.0WTE Geriatrician post will support the other half of the 0.5 CLT liaison with specific outreach to residential care units, nursing home inpatient pathway and clinical governance for Frailty front door / ambulatory support for nursing home residents in the Emergency Department. It will also provide dedicated support to 10 beds within the Sycamore Older Persons Care Unit which provides inpatient care for patients admitted from residential care. The post-holder will rotate across inpatient care, rehabilitation and outreach services to allow for enhanced integration of services within the department in a schedule to be agreed with colleagues.
