Currently, they require a clinically capable, enthusiastic, and patient-focused candidate to join their Sleep Apnoea clinical team and conduct sleep studies within the community and hospital environments. You will educate clinicians and private care home users on the correct use and practice of their products. This role will also involve evaluations, audits, and CPD sessions in the region so good presentation and communication skills are key. THE ROLE: The successful candidate will be a current registered nurse or respiratory physio. Any previous respiratory training experience is a bonus. Full product and company training will be given. The role involves visiting a variety of community and clinical settings where patients will be experiencing respiratory difficulties. You will conduct sleep studies for your patients, using sleep diagnostic equipment, then analyse the data and pass your findings on to the Respiratory Consultant managing the case. They will then diagnose the patient and recommend your suite of C-PAP machines, for which you will offer training. You will be advising patients, consultants, and nurses, delivering product demonstrations, and carrying out observations and evaluations in a patient setting. This is a field-based role so typically you will spend 4 days out in the field visiting customers and 1 day per week at home. Typical customer contact points include working with Respiratory Consultants, Community care nurses, Care home staff, Specialist nurses, etc. This is a Monday to Friday role, with flexible working hours. Clinical Focus The CNS will have a strong patient focus whereby the specialty defines itself as nursing and subscribes to the overall purpose, functions and ethical standards of nursing. Make alterations in the management of patients condition in collaboration with the MDT and the patient in line with agreed pathways and policies, procedures, protocols, and guidelines (PPPGs). Accept appropriate referrals from MDT colleagues. Co-ordinate investigations, treatment therapies, and patient follow-up. Communicate with patients, family, and/or carer as appropriate, to assess the patients needs and provide relevant support, information, education, advice, and counselling as required. Where appropriate work collaboratively with MDT colleagues across Primary and Secondary Care to provide a seamless service delivery to the patient, family, and/or carer as appropriate. Participate in medication reconciliation taking cognisance of poly-pharmacy and support medical and pharmacy staff with medication reviews and medication management. Manage and oversee treatment administration and symptom management, ensuring safe and effective care in line with best practice guidelines. Identify health promotion priorities for the patient, family and/or carer and support patient self-care in line with best evidence. This will include the provision of educational and health promotion material which is comprehensive, easy to understand and meets patients needs. Indirect Care Participate in case review with MDT colleagues. Use a case management approach to patients with complex needs in collaboration with the MDT in both Primary and Secondary Care as appropriate. Take a proactive role in the formulation and provision of evidence based PPPGs relating to oncology care. Establish and maintain clear referral line Patient / Client Advocate Communicate, negotiate, and represent patients, family, and/or carer values and decisions in relation to their condition in collaboration with MDT colleagues in both Primary and Secondary Care as appropriate. Develop and support the concept of advocacy, particularly in relation to patient participation in decision making, thereby enabling informed choice of treatment options. Clinical Nurse Specialist (CNS) Rehabilitation Service/Older Persons Rehabilitation Blarney Rehabilitation Unit and any buildings within the Mercy University Complex (Permanent, Full time) The post holder will report to: Operationally and professionally accountable to the Assistant Director of Nursing and the Director of Nursing. Clinically accountable to the Consultant Physicians in Rehabilitation Medicine. Details of Service: Blarney Rehabilitation Unit is a newly developed purpose-built post hospital rehabilitation unit. It is made up of 50 beds located off site from the MUH. It remains under the governance of the MUH. The Rehabilitation Services program delivers specialist post-acute care, focusing on patient-centered reablement to help patients achieve optimal functionality and independence following illness or injury. This service emphasizes seamless transitions of care between hospital and home, integrating multidisciplinary expertise to support patient recovery Responsibilities - The post holder will be responsible for; Provide specialist nursing assessment and interventions for rehabilitation patients. Conduct comprehensive biopsychosocial assessments and develop/implement individualized care plans in partnership with the patient, family, and MDT Independently manage a caseload of patients with complex rehabilitation needs, serving as a clinical expert in rehabilitation nursing. Coordinate investigations, therapies, and follow-up to ensure continuity of care. Act as a patient advocate and educator. Promote adaptation to lifestyle changes by teaching patients and families techniques for self-care, mobility, and daily living tasks. Encourage patient participation in goal setting and reablement activities. Lead discharge and transition planning for rehabilitation patients. Assess suitability for discharge or referral to community services and collaborate on care pathways that facilitate safe return home. Provide comprehensive patient education for home management. Establish and maintain records of patient outcomes and service indicators for the rehabilitation caseload (e.g. functional gains, service utilization). Monitor Key Performance Indicators (KPIs) as directed by nursing management. Maintain personal clinical competence in rehabilitation nursing. Stay up to date with developments in rehabilitation care and relevant research to ensure evidence-based practice. Attend courses and engage in continuous professional development. Lead formal and informal teaching activities (e.g. in-service training, ward-based learning) to enhance colleagues skills in rehabilitation care. Act as a nursing resource and role model for rehabilitation practice. Provide guidance and support to generalist nurses to enhance care quality. Demonstrate leadership in clinical decision-making and problem-solving at the ward and service level. Eligibility Criteria: Be a registered nurse on the active Register of Nurses and Midwives held by An Bord Altranais and Cnimhseachais na hireann (Nursing and Midwifery Board of Ireland) or be eligible to be so registered. And Be registered in the division(s) of the Nursing and Midwifery Board of Ireland (Bord Altranais agus Cnimhseachais na hireann) Register for which the application is being made or be entitled to be so registered. And Have a minimum of 1 years post registration full time experience or an aggregate of 1 years full-time experience in the division of the register in which the application is being made. And Have successfully completed a post registration programme of study, as certified by the education provider which verifies that the applicant has achieved a Quality and Qualifications Ireland (QQI), National Framework of Qualifications (NFQ) major academic Level 9 or higher award that is relevant to the specialist area of care (equivalent to 60 ECTS or above), and in line with the requirements for specialist practice as set out by the National Council for Nursing and Midwifery 4th ed (2008). Alternatively provide written evidence from the Higher Education Institute that they have achieved the number of ECTS credits equivalent to a Level 9 or higher standard, relevant to the specialist area of care (equivalent to 60 ECTS or above), and in line with the requirements for specialist practice as set out by the National Council for Nursing and Midwifery 4th ed (2008). And Have a minimum of 1 years experience or an aggregate of 1 years full time experience in acute setting dealing with Rehabilitation Care. And Be required to demonstrate that they have continuing professional development (CPD) relevant to the specialist area. Key Responsibilities  To give patients receiving breast care specialized knowledge, skills, and attention. To provide guidance and assistance while acknowledging the variations in the psychological effects of a breast cancer diagnosisTo guarantee ongoing, high-quality treatment for inpatients, outpatients, and their families.  To analyze, plan, implement, and evaluate all elements of the patient’s physical, psychological, and social needs.Continuously monitor patients to assess results and modify nursing care practices to provide the best possible coordination and provision of care.When necessary, the Breast Care CNS will offer pre- and post-operative counselling as well as temporary and permanent prosthesis fitting.To offer comprehensive information about treatment options, such as surgery, chemotherapy, radiotherapy, and endocrine treatment, as well as pre-and post-treatment care and supportProvide practical information on all aspects of altered image, which can be caused by the disease and treatment e.g. surgery, radiotherapy, chemotherapy and endocrine therapy.To keep accurate, legible records and be aware of the legal implications of these documents.To review Breast Care referrals in collaboration with the Lead Consultant To liaise closely with the patient’s primary healthcare teams To demonstrate the ability to participate in multi-disciplinary conferences with all professionals involved in the delivery of patient care. The Breast care CNS shall act as a patient advocate in their absence, using knowledge and skills to coordinate continuity of care.
