Overview Of Role
UPMC Hillman Cancer Centre, Waterford are recruiting a Clinical Nurse Specialist Radiation Oncology on an 11-month fixed term contract. The role is responsible for the provision of quality service in line with National and International standards. As a key member of the frontline team, the CNS will provide support and specialist advice to all disciplines of staff, patients, and members of the public regarding Radiotherapy. The position requires a strategic approach to the development of services and structures, embracing continuous quality improvement and the management of changes necessary to achieve organisational objectives.

Primary Duties And Responsibilities
Operations

  • Act as a role model through the provision of professional leadership and demonstration of best practices in Radiotherapy.
  • Act as a resource in providing specialist knowledge, consultancy, and expertise in collaboration with Cancer Centre Management, Radiation therapists, and all healthcare staff.
  • Advise on the nursing care of individual patients for Radiotherapy.
  • Maintain effective communication structures and interpersonal relations.
  • Provide education and advice to patients, relatives/carers on radiotherapy treatments.
  • Act as an advocate for the patients, and/or staff on issues in the prevention and control of infection.
  • Adhere to the Cancer Centres policies, procedures, and guidelines.

Clinical

  • The Radiation Oncology CNS will have a strong patient focus whereby the speciality defines itself as nursing and subscribes to the overall purpose, functions and ethical standards of nursing/midwifery.
  • The clinical practice role may be divided into direct and indirect care. Direct care comprises the assessment, planning, delivery and evaluation of care to the patient, family and/or carer. Indirect care relates to the activities that influence and support the provision of direct care.
  • Direct Care: Provide a specialist nursing service for patients with a diagnosis of Cancer who require support and treatment through the continuum of care.
  • Undertake comprehensive patient assessment to include physical, psychological, social and spiritual elements of care using best evidence-based practice in Cancer care.
  • Use the outcomes of patient assessment to develop and implement plans of care in conjunction with the multi-disciplinary team (MDT) and the patient, family and/or carer as appropriate.
  • Monitor and evaluate the patient's response to treatment and amend the plan of care accordingly in conjunction with the MDT and patient, family and/or carer as appropriate.
  • Make alterations in the management of patient's condition in collaboration with the MDT and the patient in line with agreed pathways and policies, procedures, protocols and guidelines.
  • Accept appropriate referrals from MDT colleagues.
  • Co-ordinate investigations, treatment therapies and patient follow-up.
  • Communicate with patients, family and/or carer as appropriate, to access patient's needs and provide relevant support, information, education, advice and counselling as required or make referrals to relevant MDT colleagues for supplementary care where required.
  • Where appropriate work collaboratively with MDT colleagues across Primary and Secondary Care to provide a seamless service delivery to the patient, family and/or carer as appropriate.
  • Participate in medication reconciliation taking cognisance of poly pharmacy and support medical and pharmacy staff with medication reviews and medication management.
  • Identify and promote specific symptom management strategies as we as the identification of triggers which may cause exacerbation of symptoms. Provide patients with appropriate self-management strategies and escalation pathways.
  • Participate in nurse-led clinics with MDT input.
  • Identify health promotion priorities for the patient, family and/or carer and support patient self-care in line with best evidence. This will include the provision of educational and health promotion material, which is comprehensive, easy to understand and meets patient needs.
  • Indirect Care: Identify and agree appropriate referral pathways for patients with cancer.
  • Participates in case review with MDT colleagues.
  • Use a case management approach to patients with complex needs in collaboration with MDT in both Primary and Secondary Care as appropriate.
  • Take a pro-active role in the formulation and provision of evidence-based policies, procedures and guidelines relating to Cancer care.
  • Take a lead role in ensuring the service for patients with cancer is in line with best practice guidelines.
  • Communicate, negotiate and represent patient's family and/or carer values and decisions in relation to their condition in collaboration with MDT colleagues in both Primary and secondary Care as appropriate.
  • Develop and support the concept of advocacy, particularly in relation to patient participation in decision making, thereby enabling informed choice of treatment options.
  • Respect and maintain the privacy, dignity and confidentiality of the patient, family and/or carer.
  • Establish, maintain and improve procedures for collaboration and cooperation between Acute Service, Primary Care and Voluntary Organisations as appropriate.
  • Maintain clinical competence in patient management within Cancer nursing, keeping up to date with relevant research to ensure the implementation of evidence-based practice.
  • Provide the patient, family and/or carer with appropriate information and other supportive interventions to increase their knowledge, skill and confidence in managing their Cancer condition.
  • Provide mentorship and preceptorship for UPMC colleagues as appropriate.
  • Participate in training programmes for nursing, MDT colleagues and key stakeholders as appropriate.
  • Be responsible for addressing own continuing professional development needs.

Audit & Research

  • Identify, initiate and conduct nursing and MDT audit and research projects relevant to the area of practice.
  • Identify, critically analyse, disseminate and integrate best evidence relating to Cancer care into practice.
  • Contribute to nursing research on all aspects of cancer care.
  • Use the outcomes of audit to improve service provision.

Consultant

  • Provide leadership in clinical practice and act as a resource and role model for cancer practice.
  • Generate and contribute to the development of clinical standards and guidelines and support implementation.
  • Use specialist knowledge to support and enhance generalist nursing practice.
  • Develop collaborative working relationships with local Cancer CNS/Registered Advance Nurse Practitioner/MDT colleagues as appropriate, developing person centred care pathways to promote the integrated of model of care delivery.
  • Attend integrated care planning meetings as required.
  • Where appropriate develop and maintain relationships with specialist services in voluntary organisations which support patients in the community.
  • Liaise with other health service providers in the development and the on-going delivery of care.
  • Network with other CNS in cancer care and in related professional associations.

Qualifications & Experience

  • Be currently registered nurse/midwife on the active Register of Nurses/Midwives held by Bord Altranais (Nursing and Midwifery Board of Ireland)
  • Have an Oncology Qualification from an accredited institution or be willing to undertake the course and in-house training.
  • Have a recognised post-registration nursing course - Higher Diploma or Post Grad course or be in pursuit of same.
  • Have extensive experience and clinical expertise i.e. minimum of 3 years (full time or an aggregate of 3 years full time) post registration experience in the specialist area of Cancer Care.
  • Have the ability to practice safely and effectively fulfilling his/her professional responsibility within his/her scope of practice.

Rehabilitation Service/Older Persons Rehabilitation
Blarney Rehabilitation Unit and any buildings within the Mercy University Complex (Permanent, Full time)

The post holder will report to:

Operationally and professionally accountable to the Assistant Director of Nursing and the Director of Nursing.

Clinically accountable to the Consultant Physician's in Rehabilitation Medicine.

Details of Service:

Blarney Rehabilitation Unit is a newly developed purpose-built post hospital rehabilitation unit. It is made up of 50 beds located off site from the MUH. It remains under the governance of the MUH. The Rehabilitation Services program delivers specialist post-acute care, focusing on patient-centered reablement to help patients achieve optimal functionality and independence following illness or injury. This service emphasizes seamless transitions of care between hospital and home, integrating multidisciplinary expertise to support patient recovery

Responsibilities - The post holder will be responsible for;

  • Provide specialist nursing assessment and interventions for rehabilitation patients. Conduct comprehensive biopsychosocial assessments and develop/implement individualized care plans in partnership with the patient, family, and MDT
  • Independently manage a caseload of patients with complex rehabilitation needs, serving as a clinical expert in rehabilitation nursing. Coordinate investigations, therapies, and follow-up to ensure continuity of care.
  • Act as a patient advocate and educator. Promote adaptation to lifestyle changes by teaching patients and families' techniques for self-care, mobility, and daily living tasks. Encourage patient participation in goal setting and reablement activities.
  • Lead discharge and transition planning for rehabilitation patients. Assess suitability for discharge or referral to community services and collaborate on care pathways that facilitate safe return home. Provide comprehensive patient education for home management.
  • Establish and maintain records of patient outcomes and service indicators for the rehabilitation caseload (e.g. functional gains, service utilization). Monitor Key Performance Indicators (KPIs) as directed by nursing management.
  • Maintain personal clinical competence in rehabilitation nursing. Stay up to date with developments in rehabilitation care and relevant research to ensure evidence-based practice. Attend courses and engage in continuous professional development.
  • Lead formal and informal teaching activities (e.g. in-service training, ward-based learning) to enhance colleagues' skills in rehabilitation care.
  • Act as a nursing resource and role model for rehabilitation practice. Provide guidance and support to generalist nurses to enhance care quality. Demonstrate leadership in clinical decision-making and problem-solving at the ward and service level.

Eligibility Criteria:

  • Be a registered nurse on the active Register of Nurses and Midwives held by An Bord Altranais and Cnaimhseachais na hEireann (Nursing and Midwifery Board of Ireland) or be eligible to be so registered.

And

  • Be registered in the division(s) of the Nursing and Midwifery Board of Ireland (Bord Altranais agus Cnaimhseachais na hEireann) Register for which the application is being made or be entitled to be so registered.

The CNS/cCNS IPC will:

  • Be professionally accountable to the Director of Nursing of Mercy University Hospital (MUH).

Purpose of the Role:

  • The CNSp Dementia Care will be required to work as a key member of the relevant specialist team.
  • They will provide comprehensive physical, psychological and emotional assessment and support to people with dementia and their families/carers throughout their disease trajectory.
  • Using a case management approach they will act as a liaison between Services for Older People and other relevant departments in Mercy University Hospital (MUH) Primary Care, Community services and other agencies in order to provide an integrated service for people with dementia and their carers/families.
  • The appointment of the CNSp Dementia Care is an essential element in the provision of effective, high quality, seamless integrated care for people with dementia.

Responsibilities Include:

  • Provide a specialist nursing service for patients with dementia that requires support and treatment through the continuum of care.
  • Provide for early identification and assessment of the patient with cognitive impairment.
  • Participate in case review with MDT (Multidisciplinary Team ) colleagues.
  • Take a proactive role in the formulation and provision of evidence based PPPGs (HSE Policies, Procedures, Protocols and Guidelines ) relating to care of the person with dementia.
  • Communicate, negotiate and represent patient's values and decisions in relation to their condition in collaboration with MDT colleagues.
  • Respect and maintain the privacy, dignity and confidentiality of the patient, family and/or carer.
  • Provide dementia awareness education, in terms of knowledge, skills and attitudes for all healthcare staff in particular the ED/AMAU (Emergency Department / Acute Medical Assessment Unit ) and hospital wards on the importance of dementia friendly and person-centred approaches to care.
  • Maintain clinical competence in the management of people with dementia, keeping up-to-date with relevant research to ensure the implementation of evidence based practice.
  • Maintain a record of clinically relevant data aligned to national KPI's (Key Performance Indicators ) as directed and advised by DON and senior management.
  • Identify, initiate and conduct nursing and MDT audit and research projects relevant to the area of dementia.
  • Provide leadership in clinical practice and act as a resource and role model for specialist practice.

Eligibility Criteria - CNS Eligibility:

  • Be a registered nurse/midwife on the active Register of Nurses and Midwives held by An Bord Altranais and Cnaimhseachais na hEireann (NMBI/Nursing and Midwifery Board of Ireland ) or be eligible to be so registered.
  • Be registered in the division(s) of the NMBI Register for which the application is being made or be entitled to be so registered.
  • Have a minimum of 1 years' post registration full time experience or an aggregate of 1 years' full time experience in the division of the register in which the application is being made.
  • Have a minimum of 1 years' experience or an aggregate of 1 years' full time experience in specialist area of Dementia.
  • Have successfully completed a post registration programme of study, as certified by the education provider which verifies that the applicant has achieved a Quality and Qualifications Ireland (QQI), National Framework of Qualifications (NFQ) major academic Level 9 or higher award that is relevant to the specialist area of care (equivalent to 60 ECTS or above), and in line with the requirements for specialist practice as set out by the National Council for Nursing and Midwifery 4th ed Alternatively provide written evidence from the Higher Education Institute that they have achieved the number of ECTS credits equivalent to a Level 9 or higher standard, relevant to the specialist area of care (equivalent to 60 ECTS or above), and in line with the requirements for specialist practice as set out by the National Council for Nursing and Midwifery 4th ed
  • Be required to demonstrate that they have continuing professional development (CPD) relevant to the specialist area.
  • Have the ability to practice safely and effectively fulfilling his/her professional responsibility within his/her scope of practice.

The purpose of this post in is to provide Specialist Palliative Care (SPC) and on-going management to patients and their families/carers under the care of OLH&CS. The aim of the role is to provide for the seamless care of this patient cohort and will involve strong clinical component together with a liaison role with the multidisciplinary team (MDT), specialty services and community services as appropriate to the care of the patient and their family / carers. The CNSp will work as a key member of the Consultant-led Inter-disciplinary team providing physical, psychological, social and emotional support to the patient and their family /carers as appropriate to patient need.

The post holder's practice is based on the five core concepts of the CNSp role as defined by the NCNM 4th edition in order to fulfil the role. The concepts are:

  • Patient/Client Advocate
  • Education and Training
  • Audit and Research
  • Consultant

Essential Requirements:
Essential Qualifications

  • Registered General Nurse with the Nursing & Midwifery Board of Ireland (NMBI), or eligible to register with NMBI.
  • Must demonstrate evidence of continuing professional development
  • Must have a level 8 (or higher) National Qualifications Authority of Ireland major academic award relevant to specialist palliative care or equivalent (contact Head of Nursing to discuss same)
  • A full current driver license and access to a carRegistered General Nurse with the Nursing & Midwifery Board of Ireland (NMBI), or eligible to register with NMBI.

Essential Experience

  • Have at minimum of 2 years post registration experience in an acute/relevant healthcare setting
  • A minimum of one years' experience in specialist palliative care
  • Have the ability to practice safely and effectively fulfilling your professional responsibility within your scope of practice
  • Competent and confident IT skills - Word, Excel, Power Point and e-mail
  • Evidence of consistent updating of clinical skills and knowledge
  • Experience in community nursing
