In line with the HSE National Programme for Progressing Disability Services , Kare is the Lead Agency for the provision of supports across South Kildare West Wicklow. The successful candidate will join an established Interdisciplinary CDNT, which reconfigured in 2014, providing quality, evidenced based and family-centred supports for children and young people aged 0-18 years, who present with complex needs associated with disability, within an interdisciplinary framework, to support their development to enable active participation in community life.
The Social Worker will support children who present with complex needs associated with a disability on Kare's Children's Disability Network Team (CDNT) and will work as a key member of the interdisciplinary team providing a family centred quality service to children and their families in line with the National Programme for Progressing Disability Services for Children and Young people. The duties will include direct support/intervention with children and

Responsibilities
Be responsible for managing and prioritising own caseload in the context of the overall service
Conduct comprehensive
assessments of children and families with a disability where required
Determine the most appropriate social work methods of practices that would best enhance the quality of life of the person with a disability
Inform the individual with a disability and their family of their rights, entitlements, benefits and services
Act as an advocate for children and families with a disability where appropriate
Liaise with Health Service Executive and relevant voluntary and statutory agencies in relation to the provision of personal and social services
To work as part of the interdisciplinary Team and engage in collaborative goal setting as a team
To participate in case formulation meetings/case reviews as required
To contribute to the planning of the service priorities as required
To be fully aware of and compliant with current child protection and safeguarding legislation and local policies and procedures
Essential Requirements
- Possess a relevant Social Work qualification (applications from recent graduates are welcomed)
- Must be registered with and maintain registration on the Social Worker Register maintained by the Social Work Registration Board at CORU
- Demonstrate the ability to ensure a high standard of service is delivered
